Responsibilities:
Thorough Chart Analysis: Examine the complete inpatient medical record in a methodical manner, including the discharge summaries, operation reports, pathology results, history and physicals, and physician progress notes.
Accurate DRG Assignment: Identify and assign the Principal Diagnosis, secondary diagnoses, and important procedures that ultimately decide the MS-DRG or APR-DRG code.
CC/MCC Identification: Accurately record the complexity and resource usage of the patient's stay by correctly identifying Complications or Comorbidities and Major Complications or Comorbidities.
Physician Query Creation: Work Together with Clinical Records When clinical documentation is unclear, contradictory, or lacking, improvement specialists should create concise, non-leading questions for doctors.
Regulatory Compliance: Strictly follow CMS rules, AHA Coding Clinic instructions, and the ICD-10-CM/PCS Official Guidelines for Coding and Reporting.
Productivity and Quality Objectives: Reach stringent daily targets for chart processing while upholding an industry-standard coding accuracy rate of at least 95%.
Discharge Status Validation: Since patient discharge disposition codes have a direct bearing on DRG reimbursement regulations, confirm them.
